Remote Exam Tutorial (Clinician Side)

Transcription
The doctor's side of a remote exam begins one of two ways.
First, you may be
provided a link by a clinical care team simply clicking
or copy and pasting.
That link will bring you into the exam room.
Second, you may log into the med wand portal.
Navigate to appointments and select Join from the task bar
on the exam you wish to enter.
Confirm the information on screen is correct. To begin.
This demonstrates common methods to start an exam.
Your clinical care team may use an alternative method,
which they will inform you of directly
once in the exam room, select start conference
to start video communication with the patient.
Once they've joined, you can manage
and mute your audio video devices with the icons
above the conference window.
You as the remote clinician will use the onscreen buttons
to operate each med wand sensor.
If the patient has joined
and the med wand is not connected,
the sensor buttons will be unusable
and will appear grayed out.
You can select a sensor using the shortcuts up top
or by selecting the sensors button.
Here we see a list of all sensors contained in the med wand,
as well as other sensors you may need to use
as part of the exam.
Let's start with med wands thermometer.
The med wand thermometer is a small hole located on the
bottom of the med wand.
The green line will help the patient get the med
wand in the right position.
Instruct the patient to hold the med wand up,
pointing the thermometer directly at the center
of their forehead, one inch from the surface of the skin.
It is very important to ensure the green line is
above the patient's eyebrows.
Instruct them to point the thermometer at the forehead,
not the eyes or nose.
When ready, select start,
which will begin a ten second countdown while the patient
takes their temperature.
Guide the patient to wave the med wand across the entire
forehead from temple to temple Twice.
Once finished, you should see the patient's temperature
displayed on screen indicating that it has been captured.
If not, try again. The med wand pulse oximeter
sensor is located on the top of the med wand.
Using the pulse oximeter,
you can record the patient's blood oxygen saturation,
also known as SP O2 value and their pulse rate.
Instruct the patient to place the index finger over the
pulse oximeter sensor.
Confirm they are covering the entire sensor
with their fingertip
and keeping their finger as still as possible.
Select start pulse oximeter
to begin recording their SP O2 and pulse rate.
If the patient's fingertip is not properly aligned,
the med wand application will report finger not detected.
You can stop the recording once satisfied,
or the recording will automatically stop after 60 seconds
and the display will show the patient's readings.
If you fail to get a reading, instruct the patient
to warm their finger
and try again as cold hands can make
it difficult to get a reading.
The med wand stethoscope is a gray disc located at the
bottom of the med wand between the ECG pads.
Using the stethoscope, you can record sounds
of the patient's heart, lungs, and abdomen.
Wearing headphones is required to properly hear these sounds
during auscultation.
Guide the patient on the proper locations
to place the stethoscope placing on bare skin if possible.
At each location, select the appropriate filter
from the right side.
Instruct them to hold the stethoscope against their body.
Once in position, press record
and ensure the patient remains still.
When you are satisfied with the recording,
press stop recording.
To save it, use onscreen
sliders to adjust loudness as needed.
You will need to turn the stethoscope off
before moving to the next sensor.
The med wand camera is located at the front of the device.
If taking images of the skin,
select Dermatoscope on the right to turn the camera on.
Guide patient to insert the supplied clear ruler into the
front slot of the med wand.
Have the patient point the med wand camera at the
location on their skin.
You want to photograph
and lightly rest the ruler against their skin.
Use the med wand application controls
to adjust image brightness as needed.
Select capture to record a high definition photograph.
To take images of the ear, select auto scope on the right
to turn the camera on.
Have the patient press a speculum into the ring
around the med wand camera.
Use onscreen arrows to center the end of the tip
to the center of the image.
You may now inspect and take images of the patient's ear.
Adjusting LED brightness and focus is needed.
Once finished, you will need to turn the camera off
before moving to the next sensor.
The med wand electrocardiogram sensor can be found
as two triangular pads located on the
bottom of the med wand.
Using the electrocardiogram, you can obtain a waveform
of the patient's heart activity.
Instruct the patient to place their thumbs on the sensor
pads on the bottom of the med one device.
Confirm that the left thumb is closest
to the cable on the pad labeled L,
and the right thumb is on the pad Labeled R.
Ensure the patient's hands do not touch
and that they sit still with their feet apart
and flat on the floor.
Select start ECG to begin a recording
and view the onscreen display of the ECG strip.
Ensure the patient keeps their thumbs on the sensor pads
and maintains even pressure.
When you are satisfied with the recording,
press stop recording to save it.
Finally, you can review all captured data
from the summary page.
To finish the exam, select save data
and exit on the summary page
or exit in the toolbar to save the exam data
to your clinical care team's records.
